A simple, compliant path to transfer embryos and donor vials between families

Seattle Sperm Bank’s Family Transfer program provides a seamless solution to coordinate embryo and vial transfer between clinics, when medically appropriate and compliant. This program provides a clear chain of custody and protects donor family limits.

Note that clinics and authorized organizations are the ONLY parties that can initiate and complete these transfers, and must include required documentation.

Start a Transfer Request

Clinics and Authorized Organizations are the only entities that can complete this process.

To begin the process, submit a Transfer Request Form with the required information. Forms must be completed by a licensed clinic or provider.

The Embryo & Vial Transfer Process

A clear, step-by-step overview of the Seattle Sperm Bank transfer review process, from initial request through final clinic confirmation.

Step 1 - Submit a Transfer Request

This is completed by the transferring clinic. Any incomplete submissions may delay the review.

  • Required Details
  • Donor number
  • Embryo ID(s), if applicable
  • Material type (vial or embryo)
  • Transferring & receiving clinics
  • Storage location
  • Donor & recipient family information
  • Chain of custody verification
  • Policy acknowledgment

Step 2 – SSB Review & Approval

Seattle Sperm Bank evaluates the request for donor eligibility, family limit availability and donor agreement compliance.

A typical review takes 7-10 business days after submission is completed. Incomplete submissions might delay review.

Note that if a donor has reached their family limit, the transfer request will be denied in accordance with donor agreements.

Step 3 - Fees & Eligibility

Family Slot / Exclusive Donors

  • Requires purchase of a Family Slot (if available)
  • Includes a $700 transfer fee
  • If unavailable, the request cannot proceed

Non-Family Slot Donors

  • Reviewed based on family limits and birth reports
  • $700 one-time transfer fee applies if approved

Step 4 – Authorization Required

Once transfer request is approved and fees are paid, Seattle Sperm Bank issues authorization.

Please note that no transfer may occur prior to approval.

Step 5 - Recipient Registration

Recipients complete the required SSB documentation to ensure proper documentation and future support.

Step 6 - Transfer Confirmation

After transfer, the clinic must confirm the final transfer details to maintain accurate records and audit compliance.

  • Transfer date
  • Material transferred
  • Receiving clinic & recipient family

Important to Know

  • SSB does not manage private financial agreements between families
  • Clinics coordinate logistics, but SSB approval is mandatory
  • Unauthorized transfers may result in loss of access to SSB services
