Specific forms may be necessary for clients and/or partners to complete before any order can be shipped out.
- Purchaser Semen and Storage Agreement (ALL CLIENTS)
- Clinic Release Form (shipments to clinics in the State of NY and the rare clinic that may require the form).
- Non-Clinic Release Form (shipments to all non-medical addresses, such as a client’s home or workplace).
- Informed Consent Form Please check your chosen donor’s webpage to see if the Informed Consent Form is required.
